Two clinical studies have evaluated the efficacy of Andrographis Paniculata extract HMPL-004 in treating mild-to-moderate ulcerative colitis in different years, both are randomized, double-blind. One is comparative, parallel-group research: HMPL-004 at dose 1200mg/day between 4500mg/day mesalazine. In this study, at this dose, HMPL-004 was determined to be similar to mesalazine in efficacy, and adverse events were rare and limited to allergic reactions (urticaria/hives). The other study, in 2013, was a placebo-controlled trial wherein patients with diagnosed ulcerative colitis (n=224), most of whom were failing first-line therapy with mesalazine, were randomized to receive either 1200 or 1800 mg daily HMPL-004 in three divided doses, or placebo for 8 weeks. This study concludes that the treatment with HMPL-004 was more effective than placebo in mucosal healing of mild-to-moderate active ulcerative colitis

A systematic review and meta-analysis included 33 RCTs(7175 patients) concludes that A. Paniculata improved cough (n = 596, standardised mean difference SMD: -0.39, 95% confidence interval CI [-0.67, -0.10]) and sore throat (n = 314, SMD: -1.13, 95% CI [-1.37, -0.89]) when compared with placebo. Among six selected candidates, purified compound andrographolide from Andrographis extract exhibited 99.9% inhibitory activity against the virus in cell cultures. Andrographis is now playing a central role in the country’s fight against COVID-19 after a series of researches in Thailand.
Phase 1 of the Thai research initiative on Andrographis started in late June 2020 at two hospitals. There, 12 patients received either 60 mg or 100 mg capsules of the extract, three times per day if they were confirmed to be infected with COVID-19 and had mild to moderate symptoms for no more than 72 hours. Results showed that the lower dose showed benefits, especially for coughing. Within three days, cough volume and overall symptom severity decreased significantly. After five days, other symptoms improved, and real-time polymerase chain reaction (PCR) tests were negative for the COVID-19 virus in two patients. After three weeks, real-time PCR tests were negative in all six subjects.
